Thus, terms will necessarily include measures of risk-adjusted clinical outcomes. In addition, to adequately account for individual patient experience and perspective, outcomes measures designed specifically to assess the patient role in care decisions and the achievement of patient goals should be incorporated. That framework, shown in Figure Figure11 and described below, can guide organizations in building value-based health care systems.
